Select the membership tier that matches the patient’s age: the Adult Plan for patients 15 and older, the Child Plan for ages 6 to 14, or the Toddler Plan for children 5 and under. If you are enrolling multiple family members, each person will have their own individual plan.
2. Complete Your Enrollment Form
Fill out your enrollment form, which outlines all plan rules and provisions. Our front office team is happy to walk you through the form and answer any questions before you sign.
3. Submit Your Membership Fee
The annual membership fee is due at the time of enrollment and covers all included services for one full year from the date of purchase. We accept most major payment methods, and our team can help you determine whether HSA or FSA funds may apply toward the cost.

The Healthy Smiles Savings Plan cannot be combined with dental insurance. Does my membership plan renew each year automatically?
Membership fees cover services for one year from the date of purchase. At the end of your membership year, you will need to renew to maintain your benefits and continue receiving the included preventive services and treatment discounts. Our team can remind you when your renewal is approaching so there is no lapse in coverage.
What happens if I need a procedure that is not included in my plan?
Included services cover your preventive care essentials. For any treatment outside of those services, such as fillings, crowns, implants, or cosmetic procedures, your membership discount of up to 25% will be applied to the cost at the time of service.
